在醫療照護與社會需求裡,經常忽略心智障礙者的主體感受,與照顧者的陪醫經驗與求醫行為,進而連帶影響就醫權利。本研究旨在探究心智障礙者成長歷程中,因認知與口語能力限制,如何向照顧者表達不適感。照顧者的陪醫經驗中,所面臨的醫療困境及求醫選擇。透過照顧者選擇不同門診型態考量因素,以及實際就醫經驗,進而分析不同門診於診療過程間,提供心智障礙者治療方法與溝通的差異。本研究以深度訪談方式,訪談13位照顧者在心智障礙者成長歷程中陪醫經歷與求醫選擇。並藉由圖文輔助,訪談心智障礙者,讓5位心智障礙者表達疾病不適感與醫療人員互動樣貌。同時,訪談3位醫療組織工作人員,闡述他們觀察心智障礙者的就醫議題觀點。透過不同門診型態的實際經驗與之間差異性,呈現現階段心智障礙者醫療需求。

In healthcare and social services, the lived experiences of individuals with intellectual disabilities and the healthcare-seeking behaviors of caregivers are often overlooked. This study explored of the healthcare seeking experiences by individuals with intellectual disabilities. Interviews were conducted with thirteen caregivers, five individuals with intellectual disabilities, and three healthcare organization staff members. The findings reveal that individuals with intellectual disabilities are unable to fully articulate their experiences and heavily rely on caregivers in the healthcare-seeking process. Caregivers prioritize healthcare choices based on accessibility to medical facilities and the kindness of physicians. Standardized medical processes easily excluded the needs of Individuals with intellectual disabilities, leading to disruptions in medical tests or treatments. Integrated and specialized clinics are designed to provide healthcare teams with expertise and relevant experience in disability. However, compared to general outpatient services, integrated clinics offer limited monthly appointment slots, imposing time constraints on healthcare arrangements for individuals with intellectual disabilities.
